REQUEST FOR STATEMENT OF QUALIFICATIONS

Emery County is seeking Statements of Qualifications (SOQ) from engineering firms licensed in the State of Utah to provide professional services for the Cottonwood Creek (Joes Valley) PL566 Watershed Project located in Emery County, Utah.

The project will be completed in three phases: PHASE ONE – Watershed Plan-Environmental Assessment (Plan-EA) Preparation; PHASE TWO – Design; and PHASE THREE – Construction. Each phase of the project will be funded individually. Phase One is currently funded. The intent is to select an engineering firm that is qualified to provide professional services for all three phases of the project.

The scope of services proposed for all three phases consists of:

• Project Management (all three phases)

• Plan-EA Preparation and Writing

• Public Outreach Coordination

• Federal, State and Local Agency Coordination (Natural Resources Conservation Service, United States Forest Service, Bureau of Land Management, Utah Dam Safety, Emery County, Orangeville City, etc.)

• Economic Analysis of Plan-EA Alternatives

• Field Surveys including Cultural Resources, Wetlands, Threatened/Endangered Species, and Geologic

• Design Analysis and Design Report

• Construction Drawings and Specifications

• Bid Schedule

• Cost Estimates

• Performance Schedules

• Inspection/Quality Assurance Plan (QA Plan)

• Operation and Maintenance Plan (O&M Plan)

• Construction Surveillance
