IN THE MATTER OF THE APPLICATION OF NORTHSTAR GAS, LLC FOR ADMINISTRATIVE APPROVAL OF THE J LEMON #1 WELL LOCATED IN SECTION 21, TOWNSHIP 20S, RANGE 7E, EMERY COUNTY, UTAH, AS A CLASS II INJECTION WELL.

THE STATE OF UTAH TO ALL PERSONS INTERESTED IN THE ABOVE ENTITLED MATTER.

Notice is hereby given that the Division of Oil, Gas and Mining (the “Division”) is commencing an informal adjudicative proceeding to consider the application of Northstar Gas, LLC, 752 S 500 E, Orem, UT, 84097-6360, telephone 435-384-2858, for administrative approval of the J LEMON #1 well, API 43-015-30070, located in SWNE, Section 21, Township 20S, Range 7E, S Meridian, Emery County, Utah, for conversion to a Class II injection well.  The adjudicative proceedings will be conducted in accordance with Utah Admin. Rule R649-10, Administrative Procedures.

Selected zones in the Navajo Sandstone Formation will be used for water injection.  The maximum requested injection pressure and rate will be determined based on fracture gradient information submitted by Northstar Gas, LLC.

Any person desiring to object to the proposed application or otherwise intervene in the proceeding, must file a written protest or notice of intervention with the Division within thirty days following publication of this notice.  The Division’s Presiding Officer for the proceeding is Dayne Doucet, Permit Manager – Oil and Gas, at P.O. Box 145801, Salt Lake City, Utah 84114-5801, phone number (385) 235-0952.  If such a protest or notice of intervention is received, a hearing will be scheduled in accordance with the aforementioned administrative procedure rule.  Protestants and/or interveners should be prepared to demonstrate at the hearing how this matter affects their interests.
