You are hereby summoned to appear before this Court in Carbon County, Utah, located at 120 EAST MAIN, PRICE, UTAH 84501, on June 4, 2021 at 1 :30 p.m. for a Termination of Parental Rights Trial involving the permanent termination of your parental rights to J. F. You have the right to be represented by counsel in these proceedings.
If you fail to appear in person at the Hearing, your parental rights to the child may be permanently terminated by the Court without further notice to you.
